AT HOME SERVICES

• We can help you organize your or your loved one's medications to makes sure the home environment is as safe as possible and that all therapy is being followed as prescribed.

• This service is helpful when medication management is time-consuming, confusing, or otherwise difficult for any reason; we make it possible for you to have a medication expert handle it for you.

• This service can be offered on a short-term or ongoing basis, depending on the patient's needs.

• A pharmacist will travel to the patient's home and will work with them to meet their individual needs.

This program provides ongoing patient monitoring, assessment, education, and support of active and emerging health issues:

• Medication check and organization​

• Lifestyle and medication compliance support

• Medication compliance packaging (eg "bubble packs" of prescriptions that are filled with us, requires opt-in) 

• Pantry checks, if making home visits 

• Telephone support outside of regular pharmacy hours

• Includes free delivery
